Questions for the discovery sprint

Participant Questions:

  1. What is your role w/in your agency?
  2. What is your interaction with forms?
  3. Do you manage the current form process?
  4. Do you make changes to the forms your group manages?
  5. How many forms does your group manage?
  6. Do you use contractors in order to make changes to forms?
  7. How are changes made to the form? Walk us through the steps you take.
  8. How costly is it to do so, in terms of time, resources, and money? (potential procurement implications)
  9. How often are these changes made (frequency/percentages)
  10. What are your biggest painpoints in managing/making changes to forms now?
  11. If you could change one thing about how forms are managed, what would that be?
  12. What works well / what doesn't work well?

Scale and process

  1. How many forms do you have?
  2. What is your most complex form?
  3. What volume (count) of forms do you process in a year? - breakdown by form number, top 5 or whatever
  4. What type of users fill them (US citizens, businesses, immigrants, government employees, etc.)
  5. How often might one user/business need to fill out one of your forms?
  6. Can most of your forms be completed in one sitting?
  7. Do you offer forms in multiple languages? Will you need to do that soon?
  8. Do the forms require payments to accompany them?
  9. Do the forms require a physical (wet) signature?
  10. Do the forms require other attachments (e.g., birth certificate or business license)
  11. Are attachments mostly for verification, or are they an extension of form information?
  12. How are forms submitted? (email, fax, postal mail, breakdown by method)
  13. How are printed forms processed once received? (scan, OCR, review/adjudication process)
  14. What percent of forms need to be returned to the applicant for correction/clarification? How long does that take?
  15. What are the most common problems encountered? (mismatched data, illegible form, missing form attachments)
  16. How much time (labor, calendar) does the department spend processing a typical good/rejected form?

Existing electronic forms (if any)

  1. Are you currently providing any electronic forms?
  2. What percentage of your data is processed via e-forms?
  3. How do your existing e-forms integrate into the paper-form flow?
  4. How do user navigate different pages
  5. What kind of validation feedback do users get when filling out the form?
  6. Do you ever need to fetch additional data in the process of completing the form?
  7. Do you use any atypical form elements for collecting information? Beyond the standard text field, dropdown, radio button, checkbox, etc.?
  8. What kinds of validations currently run on form data?
  9. Do you currently have an API for submitting and/or querying data? What format(s) does it use?
  10. Does your department/company have a strong style/branding for web sites?
  11. What are other technical requirements?
