Added support for requires_path_in_url and requires_path_in_header_named #17

Needed for galaxyproject/galaxy#16795 and elixir-oslo/gravity#2.

Should result in a new release 0.0.6 after merge (as required by elixir-oslo/gravity#2). @jchilton
