JB: Thoughts on included files in RST

See https://github.com/firedrakeproject/firedrake/pull/3309/files for an example. This needs some thought, my initial thoughts are:

• I like that this is DRY (don't repeat yourself).
• I dislike the fact that it requires manual updating every time a change is made.
• I dislike that errors in the documentation will not easily be caught.

RNH put raw code in test and put that in manual.

DH: Sphinx already has feature that allows us to use tags for inclusion; see issue

UZ: ngsPETSc Netgen hierarchy
