This PR makes all the links compatible with clicking out (either new tab or window) with the current global state.
As a result, I was also able to remove the legacy kbnUrl service 🎉
Since, every state now runs a digest on initiation we no longer have to do "$apply*" when clicking on links. This means we can getaway with using the good ol'
location.hash
andhistory.replaceState
for navigation (until we move to react router)
