Remove assets #66

For #65.

Removes all assets from RAD. This includes wURL output, twitter bootstrap design, jquery, etc.
Removes url_prefix from configuration. This allows file system browsing of HTML docs and encourages the use of Raddocs as the primary viewer.

Before this can get merged in a deprecation warning for wURL needs to be added and released as 0.9.2.

Would love to eventually get this in Raddocs as a side library, but for now it's too far of a stretch to be in RAD.

Conflicts: lib/rspec_api_documentation/wurl_writer.rb

No need to warn people about the current version.
