Focus appearance: Removing AAA SC and renaming from minumum

From the meeting today it was decided that we'd remove the AAA version, given the improvements in readability (and conveying the intent) of the AA version.

Also implementing the knock-on of renaming the SC as it doesn't include the (minimum).

Also included @mbgower's update to the focus-not-obscured SCs.
