Fix invalid changelogs / update changelog workflows

Description:

• Fix CHANGELOG.md
• Fix bug on Makefile that introduced in Vald V1 New Design APIs #826
• Update CHANGELOG template
• Update workflow & chatops for updating CHANGELOGs
  • workflow ... just using PR body for changelog details when publishing a release
  • chatops ... using /changelog command, the PR body will be replaced by the changelog detail template.
