Custom templates not being sent out

Event and notification e-mails are always sent using default template, even though custom template is being specified in version 4.1.2.

@NNovoa-CDWR what the heck?! I thought you figured out this was something specific to that first installation/server? This is occurring again on your new installation?

It was working fine and is still working on my Windows version 4.0.14 build. I've been testing the Version 4.1.2 build on two Linux servers since it was released back in October and it doesn't work anymore.
