Includes support for requesting ethereum accounts on a private chain, by providing a unique chainId and rpcUrl in a disclosure request

Adds dialog to select injected providers when present. Enables dapps to support both uPort and Metamask/Mist/Coinbase Wallet etc. as web3 providers with the same code
