urdf model converted with collada_urdf is not correct

I write the following branch for #46 . ( not sending PR yet. )

But urdf model is not good.

1. HRP2 models always fail to load to gazebo with error of fail to load urdf model generated with collada_urdf  #47.
2. SampleRobot model is loaded, but it is drawn incorrectly.

collada_to_urdf in collada_urdf may not be woring well.
