How to install the mmd_tools Blender add on
-
Download the mmd_tools add-on (powroupi fork) from here:

In the unzipped folder is a folder called mmd_tools. Copy the mmd_tools folder into Blender's scripts/add-ons folder. After opening Blender, click File, User Preferences. In User Preferences, Add-ons, Object, find the Object:mmd_tools add_on and click to put a checkmark in the little box beside it and to enable it. Beside where it says "Shared Toon Texture Folder", navigate to, and select, the Data directory of MikuMikuDance, so that the standard MMD toon textures will be used with your imported MMD models. Click "Save User Preferences" before exiting out of User Preferences.

You need to open Blender AFTER installing a new add-on. Blender detects a new add-on when it is starting up. One of the most common errors that people make when installing a Blender add-on is failing to restart Blender.
-
Also you cannot install mmd_tools directly from the .zip file. You need to copy the mmd_tools directory into Blender's scripts/addons directory.
-
Also when you are installing a more recent version of an add-on, you may need to disable it, and then re-enable it, in Blender user preferences. That step is not always necessary for every add-on. Some add-ons are programmed to reload themselves automatically.
-
Another mistake that a few people make is downloading the old version of mmd_tools by sugiany, which is no longer being developed by sugiany. The powroupi fork of mmd_tools is the version of mmd_tools which you need to download:

You should not have 2 different versions of mmd_tools installed in Blender both at the same time, because one will interfere with the other. If you have 2 different versions of mmd_tools installed, you should disable one of them in Blender User Preferences, Add-ons.
Note that after you install or re-install mmd_tools, you need to set the path to the MMD Tools Shared Toon Texture Folder:

We are migrating the documentation: MMD & Blender Wiki in English / MMD & Blender Wiki in 日本語
