Add/update CSVs for RH and partners

• Add AMQ Streams
• Add MongoDB
• Add Dynatrace
• Add Couchbase
• Add CRD permissions to aggregated roles

@robszumski I can't pull registry.reg-aws.openshift.com/openshift3/ose-configmap-reloader:v3.11.0-0.25.0.0 without logging in, which means OLM will to include a pull secret for it (and I'm not sure what to include). I think we need to figure out a better solution for operator images.

@robszumski I think we should update the instructions for these operators so that they describe the CR types and how to use them, at least.

We can go ahead and merge, but I would like to discuss criteria for an operator being listed in the catalog at some point.
