We are expecting mds_api_url is the base URL for the provider, period, and obtaining e.g. changes/trips is done by geo-query vs. a separate "feed" per provider service area, correct?
E.g. we are expecting https://mds.provider.co/ instead of https://mds.provider.co/los-angeles/ and https://mds.provider.co/santa-monica/?

This pull request adds an empty provider csv file for each operator to add in a PR.
Each company should provide their name, a UUID ID, url, and their MDS url in a following PR.
