fix: AlreadyExists Error

fixing AlreadyExists error for aws_iam_role / aws_iam_policy

updating the name of the role and policy -->
prefixing the cluster name will make the role/policy to be unique for each of the cluster.

I think this "issue" should be addressed in the codebase when the module is used by passing different value into irsa_role_name_prefix variable.
