This PR creates two Issue templates and one Pull Request template.
This PR also creates an issue template config link people to our slack server.
We could disable "blank" issues, but with the current templates, the user will still get the template found at
.github/issue_template.mdif the choose to file a "blank" issue.The general issue/default issue template encourages people to start by creating a discussion before raising an issue.
The "Action Item" Issue template is designed to be used when clear work is required.
We can itterate on these if there is feedback.
