Is your enhancement request related to a problem? Please describe.

The invidious site is quite fleshed out, but there is no manner for setting the region query parameter through preferences.

Describe the solution you'd like

It is a hassle to set the query string consistently to your preferred region when it gets reset on every hyperlink. This would be possible through userscripts, however should be an option in and of itself, available through the preferences page, as most other query parameters already are.

Describe alternatives you've considered

A userscript or API access could work, but that is simply a workaround for the missing option.

You mean that the region= URL parameter is not included in any link?

But 'Language' setting doesn't require any query, so if it will be done using cookie, url will be more cleaner, like it now is.

Also may I ask, what is the purpose of local:false cookie?

When new, not English speaking user trying to use any instance, even if (s)he set preferred 'Language' option in the settings, trending tab still include full (international?) feed (and related videos?).
Visual preferences may include one more option, and overall user experience for new users will be better.

This is a know issue. Current implementation forces the language to english (mandatory for parsing error codes) but also the region to "US", which inherently restricts the content to US channels. PR #1985 (rework of the backend API calls to youtube) is meant to fix this.
