This PR adds an option to get pre-0.75 behaviour which would leave Asciidoctor's TOC intact and it's now always removed.
I left the current default as is, but it may be worth considering if that's the correct default because it's probably not compliant with the "principle of least astonishment" (see #7649 (comment)). People coming from Asciidoctor (and probably other site generators that use it) would expect to see the Asciidoctor's TOC in the body when they add the attribute
