From @andrewwhitehead in reviewing #136.

We could be a bit more precise and say, for each witness:

Keep the latest proof from among the previous versions
Add the proof against the unpublished version, if there is one

This would mean if they didn't sign the latest transaction, there would only be one proof for that witness.
