BCeID user has two accounts?

There are BCeID users has two accounts.
One user has one active account and one inactive account. Inactive account was created before active account. Active account goes to active status without staff approve new affidavit file.
Another user has two active accounts, the second account also goes to active status without staff approve new affidavit file.

New affidavits in PENDING status.

@mengdong19 @rarya-freshworks I used BCeID john_testing500/onestop1 to test this in TEST. I could not reproduce using the following steps:

1. Create new BCeID-based account, submit and approve it as staff
2. Log into the account once it's approved and deactivate
3. Start another new account flow and use the same BCeID
4. Expected outcome - new account is automatically approved since there's already an approved affidavit tied to that BCeID.

It's possible that the account name needs to match 100%? uncertain what other variations could be attempted to try and reproduce.
