No way to stop the Jupyter server now other than reload VS Code #3508

As a user, I can get into a state where Jupyter is hosed. If restart doesn't work, I can't do anything except reload VS code to kill it.

At a minimum a command palette item would be useful. Maybe have restart actually 'restart' the entire server if it never comes back.
