Support the use case where an anchor link is above a markdown header (https://github.com/KyleKing/mdformat-mkdocs/blame/d9e9841ad5d59229c17ba4e1a635faebfd4fb6f5/tests/format/fixtures/mkdocs_autorefs.md#L1)
