Form submission

I got the calendar working but I just have one problem. I am using this calendar as a replacement for the regular <input type="date" /> for customization purposes. This is a website's book appointment page.

That being said how can I incorporate this project into a <form> tag so that on submit, the date chosen gets sent to the back end with the rest of the data gathered?

Never mind works great with bootstrap 3 v5. Thank you for your wonderful creation. If u can close this that would be great
