Cannot Add New Resource

When I try to add a new resource from either the "Add" button from the resources tab or the drop down next to the "Add Location" button. The behavior is different from each place.

1. On the Add button the dialogue area is blank
2. From the drop down no action occurs other than a "#" is added to the end of the URL
