Add support for Chlamydia in SimpleReport's single-entry and CSV Bulk Results Uploader functionalities.

Acceptance Criteria

• Develop a backend infrastructure to support the addition of the Chlamydia condition.
• Integrate the backend infrastructure with SimpleReport's single-entry functionality, enabling users to enter Chlamydia test results.
• Integrate the backend infrastructure with SimpleReport's CSV Bulk Results Uploader, enabling users to upload Chlamydia test results in bulk.
• Make the new functionality accessible to designated pilot partners only, preventing regular SimpleReport users from accessing the new condition until beta testing is complete.
• Create specific ask-on-entry (AOE) questions related to Chlamydia, ensuring they are presented to users when entering test results.
• Ensure all changes are properly documented and accompanied by appropriate tests to maintain code quality and reliability.
