Open `public/index.html` in your browser.
Go to the releases page of the Camunda modeler: https://github.com/camunda/camunda-modeler/releases/ and download the appropriate zip. Extract the zip and start the Camunda Modeler. In the new versions of Camunda (v8) the form elements have been separated from the model. So use an older versio like v7 for example.
