This PR makes modifications to the Breast Cancer screening stored procedure. Now we can target nearly all of the available indicators from the various breast screening encounters. Also, indicators are now disambiguated by prefixing their names with the encounter type name.
Also deletes an old version of the procedure that's no longer being used.
