Is there a form that will allow a subscriber to upgrade or downgrade their subscription?

Basically, we'd like for there to be a way whereby a user could, on their profile page, change their current subscription level to a different level. Something like the existing registration table, but with language tailored towards people who have already subscribed, and just want to modify their subscription.
