Alex, regarding cohort selection, here are some resources describing the conventions of Table 1 in epidemiology papers.

As I mentioned, in the "Explain It" session this week, our guest SME Dr Andrea Gropman, when asked about what kind of evidence she would like to see on results, requested being able to see the cohorts in the papers being returned.
