Add eIAM Profile Link to the login area

This issue is to address a requirement from BIT, namely to include a link to the eIAM user profile page (a.k.a. "MyAccount") from within the application once a user is logged in. Each of the eIAM environments has a dedicated URL for this:

TEST: https://www.myaccount-r.eiam.admin.ch/
INT: http://www.myaccount-a.eiam.admin.ch/
PROD: http://www.myaccount.eiam.admin.ch/

As such, the URL needs to be dynamically configurable and so, potentially solved by way of an additional environment variable, e.g. ADFS_PROFILE
