Create Template to submit application

Description

Next application cycle we want to host our own infrastructure to manage application. You task is to create a Template page that lets applicants submit their application. For now the submit button shouldn't do anything.

The form must contain the following questions:

1. Stevens Email
2. First Name
3. Last Name
4. Graduation Year
5. Which role are you interested in? (Developer, Designer, Product Manager)
6. If you are interested in development, are you more comfortable with frontend or backend?
7. Why are you interested in joining Blueprint?
8. Tell us about your proudest teamwork experience.
9. Past technical or leadership experiences?
