Populate DocumentLink's target in two steps with a new resolution step

With microsoft/language-server-protocol#401 fixed, it is now possible to store data inside a DocumentLink. We should add a new function for resolving a link so that we can populate the target in two steps instead of prematurely populating it when it may not necessarily be needed.
