follow-up to #2363
after the new api is available we can completely annihilate the old api within reason

GitMate.io thinks possibly related issues are #3083 (Remove metafunc.addcall), #3080 (Remove pytest_namespace), #3079 (Remove yield tests), #1229 (Remove plugins_index from docs?), and #3324 (how can I dynamically remove a marker?).
