Unexpected video tag in html report

I am getting unexpected video section in the report sometimes (not consistent). Following is the html tag I see:

This video tag is unexpected and it makes the report look bad. Following is my implementation inside conftest.py

Most of the time test report looks perfectly fine, this above issue only happens once in a while. Any help would be highly appreciated. Thanks.
