Running into a weird quirk where ImageOptim destroys one of my PNGs during its optimization process. Is there a way to disable lossy compression, and how do I know whether to send those instructions to optipng, pngcrush, or advpng?

middleman-imageoptim is really nothing more than some glue between middleman itself and the imageoptim gem. You would be best served to check the docs for imageoptim itself. This extension just passes config down to imageoptim.
