Actual:

foreach-value:
   &<sub>opt</sub> expression
   list-intrinsic

Expected:

foreach-value:
  &<sub>opt</sub> expression
  list-intrinsic
  [ list-expression-list ]

It's also missing for simple assignments: https://github.com/php/php-langspec/blob/master/spec/10-expressions.md#simple-assignment
