I suggest she signs two consent forms for each of the children. This is because, the lesson creation/suggestions team will also have a record of the consent forms as the forms will have your name as the facilitator, so the number of students you interviewed should tally with the number of consent forms signed. |

Hello @Amarachhhiii It is stated in the checklist that if the parents or guardians are not able to sign the consent form, due to not having device or other reasons, the facilatator should fill it on behalf of the parents using the facilitators consent form. |

@Amarachhhiii I think you should make use of the consent form for facilitators, as @successoghogho suggested, though there is a paper form option in this form. I think you can upload the same paper form signed by the participants' mother for all the consent submissions. You can go for the second option too in the form But if you're choosing the first one, you can ensure your name is included in the written consent form you're uploading. |

@successoghogho @Raffysul I don't think you understand the question I'm asking. I and the guardian already chose that she would fill in the paper form because she wanted a copy for herself to show her husband. My question is since both children are from the same guardian do I need to give the guardian two paper form for each or just one paper form for both as it's one household. |

@Amarachhhiii Since that is the case. I believe it's advisable the mother signs 2 different forms, because they are actually two students even though they have the same surname. Oppia views them as two separate individuals. That's why on your research template, you will assign two different ID to them. |

@Amarachhhiii , In my opinion, since you will be conducting the test with two students, there should be separate forms for both students. I understand your concerns with regard to having the same parent fill out the same form twice. I'd say you should try to explain to her that both participants will have separate participant IDs, hence the need for the separate form. I believe she will understand. |

You can have the parent fill one consent form for them. In your report, just state somewhere that one consent form was filled for the two participants, since they are from the same parents. |

I've been on Task 2 and I'm just about to begin my Learner Feedback task and I am faced with a confusion that I need clarity on. I have spoken with the mother of the students(who are within the ages 7-13) I plan on using for this study, she has accepted and we have established that she would have to sign the paper form as she doesn't have any smart device nor does she have an email address.
Here is my question is, since she is the mother of the children I want to use, would she sign two consent forms for both children or just one for all of them since it's the same household? I'm asking this because I noticed there was no place for us to fill in participant ID in the form. Do I give her one consent form or two consent form.
