Update all the SRO measures codelists

The SRO measures¹ codelists were created around April 2021. They therefore need to be checked for updated codes. This includes the following codelists:

• Alanine aminotransferase (ALT) tests
• Asthma annual review QOF
• CVD risk assessment score QOF
• Cholesterol tests
• Chronic obstructive pulmonary disease (COPD) review QoF
• Glycated haemoglobin (HbA1c) tests
• Red blood cell (RBC) tests
• Sodium tests (numerical value)
• Systolic blood pressure QoF
• Thyroid stimulating hormone (TSH) testing

A lot of these codelists are easy to update with the Create new version button. However three of the codelists come from QOF and were presumably created by uploading a CSV. This means that they don't have any associated search terms, and pressing Create new version doesn't search for anything new.

Affected codelists are:

Asthma annual review QOF
CVD risk assessment score QOF
Chronic obstructive pulmonary disease (COPD) review QoF

We could either treat these codelists as canonical as they're from an external source, or attempt to update them regardless.
