Establish semantic convention for "file" attributes and ensure they are followed in this library

Not quite. We've established semantic conventions but aren't using them in this library yet.

Given that we have at least some obligation to minimize breaking changes, I believe we should plan on cutting a v2 of the library in Q1. Besides the full resolution of this issue, it would include several other breaking changes relating to recent data model decisions. I'm working out a proposed set of changes, which I plan to share in early January.
