Express Instrumentation Request Handler Timestamp

I have a question regarding instrumentation (ESM). The @opentelemetry/instrumentation-express and @opentelemetry/instrumentation-http packages are installed and enabled (defaults). However when I observe the length of the express instrumented route type span, things are not to my expectation. I placed a 20ms wait in the get route (code seen bellow). I expected the elapsed time to show itself in the service1: request handler - /fetch/host span, but it does not. That span being 12us. However, the 20ms is added to the top level http instrumented span.

Is this correct or do I have a misconfiguration?
