Specify width or columns on each oxy_combo CSS form

We should specify either a width (which can be set in em or ex) or a number of columns on each oxy_combo CSS form control.
Not setting the width/columns can lead to performance problems when editing largish TEI files.

I added width in one CSS but there are others as well which would need to be changed.
