Restore header describing the commit hash of reference report

In the noir-gates-diff action we have a header which describes which commit the report is generated for and the commit which this report is being compared against. This was removed from this repository for some reason, although it's actually very useful for determining whether the report is up-to-date.

We should pull across this logic and add it into the report here as well.
