EyePattern: add BER data stream output

The current "hit rate" is defined according to SFF-8431 as (mask hits) / (samples integrated).

We should also have a "Mask BER" output which respects UI boundaries: any number of mask hits between two PLL clock edges are considered a single bit error; output (UIs with bit error) / (total UIs integrated).
