Tento týden se zaměříme na situaci v Asii. Popíšeme řadu oddělených, samostatných případů zanesení nákazy do Indie, souvislosti týkající se propuknutí nákazy mezi migrujícími pracovníky v Singapuru a převažující charakter mezinárodního šíření po regionu.

# Úvodní přehled

Provedli jsme analýzu 2 226 veřejně dostupných genomů viru způsobujícího nemoc COVID-19. Vzájemným porovnáním těchto virových genomů můžeme popsat, jak původce nemoci COVID-19 postupuje světem a šíří se v rámci různých lokalit. Novinky tohoto týdne se zaměřují na šíření v Asii. Popisujeme v nich následující události:

* Mnoho případů zanesení nákazy z celého světa do Dillí během března 2020  
* Mnoho oddělených případů zanesení nákazy do Singapuru, z nichž většina se patrně dále nešířila  
* Rozšíření velkého klastru případů v Singapuru, pravděpodobně vyvolané sociologickými faktory
* Podobné modely mezinárodního šíření, které zdůrazňují naléhavou potřebu spolupráce

Poznámka k odběru vzorků

V současnosti máme k dispozici sekvence ze vzorků odebraných v 28 zemích v Asii. To je neuvěřitelný úspěch – sekvenování neznámého, velkého RNA viru uprostřed pandemie je obtížné a lze jej provést pouze díky neskutečné práci a včasnému sdílení dat vědci a lékaři z celého světa.

I když nám tato data umožňují vyvodit mnoho užitečných charakteristik propuknutí nákazy a sledovat její šíření v reálném čase, je důležité zdůraznit, že naše závěry jsou limitovány dostupnými údaji.

Mapa například ukazuje relativně málo sekvencí ze Středního Východu. Důvodem rozhodně není to, že by se nemoc COVID-19 snad v těchto oblastech nešířila nebo že by zjištěné případy nebyly natolik zásadní, abychom se jim snažili porozumět. Z těchto oblastí především nemáme k dispozici dostatečné množství údajů. Velikost jednotlivých kruhů na mapě tedy neoznačuje skutečný rozsah nákazy, ale spíše kolik dat je aktuálně z dané oblasti k dispozici.

Mnoho samostatných případů zanesení nákazy do Dillí během března

Mezi 1. březnem a 15. dubnem bylo v metropolitní oblasti Dillí odebráno a sekvenováno 38 virových genomů (viz zde).

Uprostřed stromu vidíme velký klastr úzce příbuzných případů. To odpovídá předpokladu o jediném případu zanesení, po kterém následovalo šíření mezi místním obyvatelstvem.

Nesmíme však zapomenout vzít v úvahu všechny souvislosti tohoto klastru.
Po celém stromu vidíme také roztroušený výskyt mnoha dalších malých klastrů a ojedinělých případů, které představují samostatné případy zanesení nákazy z celého světa. Nemáme dostatek údajů k tomu, abychom mohli říci, co následovalo po těchto dalších případech zanesení – zda nebo jak dalece se rozšířily. Můžeme však prohlásit, že zmíněný velký klastr případů představuje pouze část celého příběhu.

Pozadí opětovného nárůstu počtu případů nákazy virem SARS-CoV-2 v Singapuru

Během posledních 10 týdnů byl virus SARS-CoV-2 zanesen do Singapuru mnohokrát a z různých míst.

Většina těchto případů zanesení vedla k omezenému dalšímu přenosu (jednotlivé případy nebo malé klastry na stromě).

Uprostřed stromu však vidíme jediný případ zanesení, který vedl k rozsáhlému následnému šíření. Tento velký kompaktní klastr je nejpříbuznější se vzorky z Indie a Filipín.

Tato pozorování jsou v souladu s nedávnými zprávami o tom, že se virus v Singapuru rychle šíří mezi zranitelnými skupinami migrujících pracovníků žijících ve stísněných podmínkách.

Tento závěr ukazuje, jak je rozsah šíření v rámci populace po prvotním zanesení nákazy závislý na sociologických faktorech, a zdůrazňuje, proč je zásadně důležité chránit zranitelné skupiny.

Opakované zanesení nákazy do Malajsie

Na webu Nextstrain máme 14 sekvencí z Malajsie. Jednotlivé vzorky byly odebrány od konce ledna do konce března. Tyto sekvence se shlukují s několika různými klady, z nichž každý se vyskytoval většinou v různých asijských zemích. To napovídá, že do Malajsie byla nákaza z okolního regionu zanesena opakovaně.

Regionální šíření po celé Asii

Podobný model se opakuje v řadě dalších zemí regionu: vícenásobné zanesení nákazy, přičemž většina případů zůstala dobře izolovaných, ale několik málo spustilo místní přenos. Doporučujeme vám, abyste si data prozkoumali sami v části „Explore the Data Yourself“. Jak je zde ukázáno, podobné charakteristické struktury budete pozorovat v Jordánsku, Japonsku, Thajsku a ve Spojených arabských emirátech. Obdobné struktury vidíme také v Kuvajtu, Gruzii, Tchaj-wanu a Vietnamu.

Toto opakování je jasným důkazem toho, že virus často překračuje mezistátní hranice. Abychom tedy mohli epidemii účinně kontrolovat v jedné zemi, musíme pomocí koordinovaného mezinárodního úsilí dostat pandemii pod kontrolu ve všech zemích.

Co můžete udělat vy

... jako běžný občan

• Důsledně omezovat společenský kontakt, zejména pokud spadáte do ohrožené skupiny.
• Nezapomínejte, že i když vy sami do silně ohrožené skupiny nepatříte, řada lidí kolem vás ano. Dodržováním těchto opatření chráníte ostatní.
• Myjte si ruce, „jako byste právě nakrájeli silně pálivou papriku a chystali se vyměnit si kontaktní čočky“.
• Co nejvíce se zdržujte doma – zejména pokud jste nemocní. Pořiďte si zásoby pro případ, že budete muset zůstat v domácí karanténě.
• Jste-li zaměstnavatel, nabídněte všem zaměstnancům, u kterých je to možné, práci z domova.

... jako představitel veřejné moci

• Zpřístupněte bezplatné a široce dostupné testování.
• Zaveďte přísná pravidla omezení společenského kontaktu (tzv. social distancing).
• Financujte a zavádějte rozsáhlé projekty trasování kontaktů.
• Podpořte finančně osoby postižené pravidly omezení společenského kontaktu.

# Klíčové závěry

#### Propuknutí nákazy v Indii je důsledkem řady samostatných případů jejího zanesení z celého světa.  
<br><br>
#### V Singapuru urychlily sociologické faktory rozšíření na základě jednoho případu zanesení, z něhož se následně vyvinulo velké ohnisko nákazy. Oproti tomu došlo i k řadě případů zanesení, které se výrazně nerozšířily.
<br><br>
#### Ohniska ve většině asijských zemí mají podobnou historii: mnoho samostatných případů zanesení nákazy z jiných zemí, z nichž některé vedly k místnímu šíření. Tento model opakovaného zanášení v průběhu času zdůrazňuje potřebu mezinárodní a regionální spolupráce při účinném zvládání pandemie.

Chtěli bychom ocenit úžasnou a rychlou práci všech vědců bojujících s touto nákazou. Analýzy tohoto typu jsou možné pouze na základě rychlého sdílení genomových dat a metadat.
