NICs for ESI Node MOC-R4PAC24U27-S3 do not come up

First noticed when Eric provisioned MOC-R4PAC24U27-S3; the network interfaces never came up, making it impossible to connect to the node. I checked the switch and the vlan and port were configured correctly; however there was no entry in the MAC address table. When I rebooted the node to look at the startup log, I noticed that the network interfaces were not coming up.

When I tried to clean the node, the node attempted to PXE boot; however the network interface again refused to come up, and it ended up booting from the hard drive.
