Ending the process (CTRL+C) in terminal window does not send terminate event #82

If I'm not mistaken, previous version sent the exited and terminated events.
This is no longer happening, this leaves the VS Code debugger (in an invalid state), seems to be running even though the debugee is dead.

I stand corrected, nothing has changed.
Same behavior with the old debugger
