Missing indexes on spammers table #70

I have noticed that there are no indexes on the spammers tables and there is no easy way to add them.

I suggest that the library either adds the indexes by default or allows the publishing of the migrations so that the user can change them.

For now, I will just create a migration that adds those indexes with the hope that Laravel handles the order properly.
