User teams in user-list and on dashboard

Description

• Show user avatar in upper right menu - Fixes Profile image #1078
• Added avatars in user list
• Added teams in user list - Fixes User view to list the teams names where the user is assigned as a user and as a team lead #1146
• Added new widget: "my teams" on dashboard (see screenshot)
• Added new widget: "Team projects & budgets" on dashboard (see screenshot)
• Added library to generate personal default avatars
• Added "Doctor" controller to verify several system settings, file permissions, display log file
