Subjective Judging Score Entry - Provide visual distinction if Rubric not evaluated #969

Scenario:

As a Judge, it is not easy to distinguish between a very poor score (1) and failing to evaluate a particular rubric

Description:

while not common, it is a team could receive such a poor score.
If no rubric is evaluated, it would be very obvious.
Particularly on small tablets, it might not be immediately obvious that all rubrics have been evaluated.
Also, it is possible to click 'Save changes' before all rubrics have been evaluated.

Options:

Add an unscored (0) entry that doesn't correspond to a rubric category (no green highlight)
Require comment if poor score (1) is given
Highlight on Score Summary page if any team has a rubric scored as 1
