Creating a patient Model

• This feature will provide space for the patient to provide its condition and information. The logged in user which is a patient can give update or create their information for the doctor to review it.

To Reproduce

• Steps to reproduce the behavior:
  1.Make a model folder and its schema.
  2.A entry would be there for clear and detailed information
  3.Schema would have created information timing.
  4.Reviewed doctor cam comment on the information and reviews of doctor would be present and visible for patient.

Expected behavior

• A patient can inform and take the reviews of doctor

Hey @apeksha235 , patients information of their cure and disease update/lab reports. It will also be containing the effect and feedback of the their ongoing treatement. We can also add a section for the doctor to tag who is treating them.
