Weird! Is this on Android? I can't repo on Chrome for iOS or Mac.

It was chrome on mac.

I pushed a change that adds autocomplete="off" to the input fields (and the form just for good luck). Hopefully that should be enough to fix it, but I won't close this out yet because I'm not 100% sure of the fix.
